Background – Understanding JC’s Needs
JC, aged 22, lives in Sussex with his mum, Tracey, and their dog, Marley. He has SWAN (Syndrome Without a Name), a condition that causes epilepsy, unsteady gait, ADHD, and severe learning difficulties.
Despite these challenges, JC is full of energy and enjoys staying active – from trips to the park and bowling to swimming and spending time at the arcade playing his favourite 2p machines. He also loves music and has a wide-ranging taste in songs.
Our Approach – Putting the Right Support in Place
When we first met JC and his family, it was clear that a tailored care package was needed to meet his individual needs and provide Tracey with the consistent support she deserved.
Working closely with them, we designed a care plan focused on enabling JC to stay active, engaged, and safe at home and in the community. The process went smoothly, and JC now has a 24/7 care team that supports him to do the things he loves while helping him maintain a structured, fulfilling routine.
