Libertatem’s care model is clinically led from referral through to mobilisation and day-to-day delivery. Experienced nurses and clinical leads are embedded throughout the sales process, client onboarding, and ongoing service oversight, ensuring a consistent clinical voice and proactive risk management across all touchpoints
Structured governance oversight
Tiered governance meetings: action plan reviews, risk register reviews, and quality reviews are reviewed monthly at the Governance Forum. Clear governance chain including escalations managed through a defined structure – supporting joined-up accountability and system-wide learning. A biannual audit framework is tracked via Radar, with findings reviewed thematically and actioned through cross-functional plans.
Libertatem maintains rigorous internal governance protocols, including regular clinical audits and compliance reviews, ensuring high standards of care and operational integrity.
Local and central risk management – Risk is managed and owned locally by field teams who are trained to recognise risks as they emerge. Clients are proactively risk profiled upon starting a new package, helping to manage potential risks. Clear crisis and escalation protocols are established with carer and nursing teams. Libertatem follows a clear hospitalisations management and avoidance framework to keep clients where possible at home which is better for client and commissioner. Underpinned by a commitment to recruiting and training of staff to drive underlying principles of quality at all organisational levels.
Personalisation: care built around the individual – Bespoke care teams recruited for each client, ensuring the team supports specific needs and preferences. Local carers, managers and nursing staff are embedded into the client’s lives becoming trusted by their families and support networks.
Outcome-focused personalisation: care is centred on what the client wants to achieve including mobility goals, communication targets, reintegration to returning to work or improving social engagement
Quality through consistency – Effective recruitment and retention of high quality staff is paramount to care quality at both a local and regional level
Supervision of work and review and appraisal frameworks support staff in their roles to ensure consistency of care. To aid standardisation and mobilisation, digital care plans and risk assessments are stored in OneTouch, reviewed regularly, and overseen by senior nurses and Registered Managers
External organisations verify the quality of the service provided at Libertatem:

- CQC (England): Compliant; rated “Outstanding” in August 2025 – second consecutive inspection
- CIW (Wales): Compliant; rated “Good” in September 2025
- RQIA (Northern Ireland): Compliant; inspected in October 2025 RQIA does not provide quality ratings.
- The Clinical Director serves as the Nominated Individual, with region-specific Registered Managers overseeing local compliance.
